- Lead the successful launch and regulatory accreditation of the Hospice program, including CMS certification and Joint Commission accreditation.
- Develop, refine, and maintain policies, procedures, and interdisciplinary workflows to ensure full compliance and operational consistency.
- Ensure survey readiness at all times through proactive monitoring, internal audits, and QAPI participation.
- Quality, Safety & Performance Improvement
- Develop and lead QAPI programs across Home Health and Hospice; monitor quality metrics and drive continuous improvement.
- Evaluate clinical, operational, and financial data to guide decision-making and enhance outcomes.
- Support initiatives that improve patient satisfaction, care coordination, productivity, and service effectiveness.
- Financial & Performance Management
- Partner with executive leadership on budgeting, forecasting, and financial performance across both service lines.
- Monitor KPIs, productivity benchmarks, and cost performance to ensure sustainable program growth.
- Collaborate with leaders to implement workflow improvements and operational efficiencies that support strong fiscal health.
- Program Development & Strategic Execution
- Build scalable clinical and operational systems to support growth, quality, and organizational readiness.
- Lead foundational development of the Hospice service line during the start-up phase, including staffing, workflows, scheduling, intake, coordination, and documentation.
- Transition operational oversight by hiring and mentoring a Clinic Manager as the Hospice census grows.
- Support systemwide initiatives related to compliance, technology adoption, performance measurement, and data-driven decision-making.
- Referral, Community & Market Growth
- Strengthen relationships with referral partners, healthcare providers, and community organizations to support census growth and expand market presence.
- Represent Company within the community and industry to elevate brand visibility and foster trust.
- Support marketing and outreach initiatives that promote Home Health and Hospice services.

Position Overview: Reporting to the Senior Vice President of Operations, the Executive Director serves as the senior clinical and operational leader for both Home Health and Hospice. This highly visible role provides executive oversight of care quality, regulatory compliance, staffing, financial performance, and strategic growth across both service lines . During the Hospice Start-Up Phase, the Executive Director will play a hands-on, build-from-the-ground-up leadership role, guiding CMS certification, leading The Joint Commission accreditation, establishing clinical workflows, and developing an interdisciplinary team capable of delivering exceptional end-of-life care. Simultaneously, this leader ensures the Home Health program continues to operate at a high standard, partnering closely with the SVP of Operations and the Administrator to maintain regulatory readiness, optimize operations, and support team development and retention .
