- Responsible for establishing the TapestryHealth patient relationship for our Telehealth, Vitals Management, and other services.
- Available for a minimum of 10 weekdays per month to conduct approximately 375 Initiating Patient Visits onsite and remotely.
- These are brief, non-clinical judgment, educational consults simply advising patients of the services they are enrolled in.
- Tapestry Health will reimburse for mileage as well.
- Document patient encounters accurately and thoroughly within our platform at time of visit.
- Provide end of day report to program administrators to track progress for completion of visits.

- Familiarity with electronic health record (EHR) systems and healthcare technology.
- Ability to travel within 1-2 hours of assigned region
