Responsibilities
- Delivery of Patient Care
- Assumes responsibility for the coordination and delivery of patient/caregiver care.
- Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
- When appropriate, develops, implements, evaluates, and updates the plan of care to ensure that care is uninterrupted and that goals are achieved.
- Facilitates patient and caregiver independence to the extent possible.
- When providing hospice care, appropriately utilizes VIA Health Partners patient related protocols, standing orders and guidelines.
- Assumes responsibility for conducting effective patient and caregiver education.
- Encourages and allows time for questions and consultation.
- Provides education to patients and families as needed.
- Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.
- Maintains open lines of communication with supervisor.
- Keeps supervisor apprised of any complex patient/caregiver issues.
- Applies principles of infection control.
- Follows guidelines for universal precautions in accordance with accepted professional practices.
- Interdisciplinary Group
- Attends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences.
- Assumes responsibility for establishing and maintaining effective working relationships with team members.
- Identifies specific patient/caregiver needs and make referral to appropriate team members.
- Documentation
- Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.
- Professional and Personal Accountability
- Demonstrates competency in knowledge and skills.
- Ensures that VIA Health Partners professional reputation is maintained and projected.
- Assumes responsibility for professional development and staying abreast of current trends in the healthcare field.
- Incorporates new information and methods into practice.
- Maintains current professional licensure, CPR certification and CEU requirements.
- Maintain productive and professional relations with external agencies and healthcare providers.
- Exhibits time management skills.
- Able to plan workday while remaining able to respond appropriately to crisis.
- May perform other duties as required
- (IPU) Cleans patient rooms as required per patient room cleaning procedure.
- Ensures all areas of facility are clean and appropriately stocked with supplies.
- Keeps the family room clean throughout the day.
- Ensures proper chemicals are used for cleaning different surfaces.
- Ensures protective equipment is used when working with chemicals, i.e., gloves, glasses.

Requirements
- A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
- The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.
- Current CPR required; CHPN preferred.
- Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions.
- Experience in direct care of patients with life limiting illnesses required.
- Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
- Computer proficiency is required
