Responsibilities
- Assessment and Care Planning: Conduct comprehensive assessments of clients' health status, including physical, emotional, and psychosocial factors, to develop individualized care plans.
- Skilled Nursing Care: Provide skilled nursing care and interventions, including wound care, medication management, IV therapy, catheter care, and other nursing procedures, as prescribed by the physician.
- Patient Education: Educate clients and their families on disease management, treatment plans, medication administration, and self-care techniques to promote independence and improve health outcomes.
- Care Coordination: Coordinate and collaborate with other members of the healthcare team, including therapists, social workers, and home health aides, to ensure seamless continuity of care and optimal client outcomes.
- Documentation: Maintain accurate and up-to-date documentation of nursing assessments, interventions, and client progress in electronic health records (EHR) or other documentation systems, in compliance with agency policies and regulatory requirements.
- Medication Management: Administer medications, monitor medication effectiveness and side effects, and ensure adherence to medication schedules, as prescribed by the physician.
- Patient Advocacy: Serve as an advocate for clients and their families, ensuring their needs and preferences are communicated and addressed effectively by the healthcare team.
- Safety and Infection Control: Implement infection control measures, assess safety hazards in the home environment, and educate clients and caregivers on injury prevention and safety practices.

- Current unrestricted state licensure as a Registered Nurse (RN).
